Clergy, spouses and lay employees participating in the HealthFlex active health insurance plan (not retirees) are encouraged to sign up today for the BluePrint for Wellness health check during Annual Conference.
The results will provide you the information you need to complete your HealthQuotient by July 31, and avoid paying a higher deductible on your health insurance in 2020.
Register online through your HealthFlex/WebMD account at Wespath.org. Select “Quest Diagnostics - Blueprint for Wellness,” click to register “At an event,” and search for the KI Convention Center in Green Bay June 15 and 16, 2019. Or you can make an appointment by calling 1-855-623-9355 and mention that your employer group is HealthFlex or the United Methodist Church. If you would prefer to do the testing at a time and place other than Annual Conference, there are testing sites available throughout the state and across the state lines in Illinois and Minnesota. Go to https://appointment.questdiagnostics.com/patient/findlocation to find a location and make an appointment at a testing site.
Walk-ins will not be accepted. Please register as soon as possible before June 1, 2019.
There is no cost to participants if this wellness test is taken at Annual Conference or if you register at a Quest Diagnostics facility. The 2019 Incentive for participating is $100 PulseCash for those registered with Virgin Pulse and up to 120 Wellness Points.
