Elder and Disability Services assist persons who are unable to pay the full cost of services provided, as well as persons with sufficient financial resources. Contributions through the annual Golden Cross offering are used to offset the cost of uncompensated services. Services in this area include: skilled nursing care, assisted living, home care, hospice care, home delivered meals, senior center, rehab and therapy services, transportation, fitness and aquatic facilities, and spiritual services.

Cedar Crest, Janesville

Cedar Crest is Rock County's only community that provides senior adults with this range of living options, including town homes and apartment homes, assisted living studios, memory care residence, and a health center. Cedar Crest has approximately 250 residents, and strives to enrich the quality of life for senior adults at all levels of independence in a spiritually-based, nonprofit setting that features compassionate care. Christian Community Home, Hudson

With campuses in Hudson and Osceola, Christian Community Homes provides resident senior care, assisted living, and skilled nursing facilities. As a church-sponsored organization, its mission is to serve residents in a loving and caring environment. Its leaders, staff and volunteers are dedicated to helping residents meet their physical, social, emotional, and spiritual needs, and to encourage a sense of dignity and individuality. Evergreen Retirement Community, Oshkosh

Guided by Christian values, Evergreen is a nonprofit continuing care retirement community serving older adults, ages 55 and older, and serves more than 500 people annually through its current campus, home care, and Meals on Wheels programs. Short-term rehab and hospice care are available, and through a private partnership services are provided to residents of a new affordable housing project. (Fund No. 7110)

Morrow Home Community, Sparta

The Morrow Home Community serves as a tangible expression of God's love through a continuum of long-term care services, including senior apartments, assisted living and dementia care. Mary Morrow's Attic, a thrift store run by volunteers, operates in downtown Sparta. Morrow also provides daycare at the Young at Heart childcare center. Morrow Home strives to enhance quality of life by preserving the dignity, independence, and uniqueness of each individual while meeting their physical, social, and spiritual needs. (Fund No. 7320)

Schmitt Woodland Hills, Richland Center

Schmitt Woodland Hills is home to an active group of seniors who share the benefits of a welcoming, secure, and comfortable lifestyle. Services include skilled nursing care, assisted living, independent apartments, personal supportive home services, and non-emergency medical transportation. They also partner with a 19-unit HUD low-income apartment facility, a children's day school, and Pet Therapy. They provide services to more than 200 people. Called to create a Christian environment, Schmitt Woodland Hills maintains a pastoral care staff and a designated chapel. (Fund No. 7840)

Sheboygan Senior Community, Sheboygan

Sheboygan Retirement Home serves the social, spiritual, and health-related needs of seniors in downtown Sheboygan through a continuum of living options, including assisted living, skilled nursing, rehabilitation services, and memory support. (Fund No.7270)

Village at Manor Park, West Allis

Village at Manor Park (VMP) of Milwaukee is a residential and outpatient senior health care community that provides independent, assisted living, and skilled care. VMP also provides rehabilitation and ventilator care. The Senior Center is open to the community and currently has 1,300 members. VMP offers “Super Noggin,” a series of community workshops that focus on healthy eating, exercise, and brain teasers to promote prevention of Alzheimer's Disease. (Fund No. 7710)
